For the new location: signed lease agreement with dates of residency (if renting) ORproof of purchase (i.e. bill of sale, cash deed warranty, or property tax notice, etc.) if owning.
911 Permit Number for the new location - you can get this by calling the 911 Office at 337-238-9911.
All unpaid balances must be paid before or at the time of the transfer request.
CLOSING AN ACCOUNT:
It is the account holder's responsibility to notify the city when you wish service to be discontinued. If you fail to discontinue service, you will continue to be responsible for any/all charges accrued, even if you move out. Retroactive requests to discontinue service will not be honored.
